I haven't done a search on it, but I'm sure it's out of print. However, I think
the following is still in print: it's a more-then adequate substitute for the
above, and something belongs on everyone's (well, everyone interested in
tuning's):

A.D.Fokker, Selected Musical Compositions 1948-72 (Diapason Press, Utrecht,
1987).

It's a beautiful book, with extended biographical and theoretical material by
Rudolph Rasch. The Diapason Press/Huygens-Fokker Foundation web site should have
the purchasing information.

Thank you, Daniel Wolf, for the reference. I had no idea even that
Fokker was a composer! I will look into it...

Joseph, we have it in the AFMM archives. Fokker composed under the name
"Arie de Leon." (unsure of the spelling)
