i would suggest VMCI which is in fact Real-time.
Darren and i have been using it here at SEJIS
You can (via some examples) tune your instrument however you like
The Piano sound may be something you may need to work on but you can start
with a good .wav or aiff of a piano sample and manipulate it from there
