In a word: Multiplex.

For example, suppose you take your cue from the guitar in which notes are
selected with the left hand and sounded from the right. If you had 5 keys on
the left hand, you could have 32 combinations of keys being down or up. If
you had 10 keys on the right hand, then you would have the possibility of
sounding 320 notes.

Adding some more keys, or adding a state machine to the hardware or software
could add more possibilities. These are just standard tricks for designing
electronic user interface circuitry. The hard part is getting musicians to
accept a new "standard." For that, I have no answer.
