I think Cakewalk has a sort of gollywog mind. Every now and then,
well, halfway between now and then, when it plays a MIDI file as it's
downloaded, it transposes it up a step. It plays it in the proper key when
next attempted. I dunno.

I'm who experienced the Cakewalk slowdown of John deL's Mozart retuning,
and have had other odd things happen with MIDI tempi in Cakewalk, but never
experienced pitch transpositions as you describe below. I'm still looking
for a solution to the tempo problems (hopefully short of abandoning
Cakewalk), and would welcome any tips.

Not much 'tuning' content here, so I'll add that I am still happy with
Audio Compositor, the shareware I previously mentioned for all-digital
translation of midi to wave using whatever soundfont or instrument
definition file you choose, that also includes the capacity for retuning
from 12-tET to other fixed tunings. No affiliation but a happy customer,
